Evaluates, plans and administers occupational therapy modalities/regimens to patients in their place of residence for the purpose of developing and restoring function through rehabilitative occupational therapy as prescribed by a physician. Provides supervision of occupational therapist assistants.

 

Responsibilities

 

    • Performs accurate and comprehensive assessments.
    • Accurately completes OASIS data sets.
    • Obtains physician orders for certification and recertification of home occupational therapy services.
    • Ensures that changes in patients’ clinical status are communicated to other healthcare team members and HHA office staff verbally and in writing in a timely manner.
    • Ensures that assessment results, recommendations, and physician orders are communicated to other healthcare team members and HHA staff in a timely, accurate and thorough manner.
    • Coordinates care planning and delivery with the physician and other healthcare team members and HHA staff to facilitate optimal patient outcomes.
    • Delivers occupational therapy modalities appropriate to the patient’s clinical status and needs, and in accordance with the standard of care for patient’s age and with applicable laws, regulations and standards.
    • Assesses the efficiency and effectiveness of occupational therapy services and designs and implements processes to support optimal patient outcomes.
    • Develops appropriate home exercise programs tailored to the patient’s clinical status, needs and age.
    • Able to appropriately assess and reassess a patient’s pain.
    • Utilizes appropriate pain management modalities that include, but are not limited to, occupational therapy modalities.
    • Provides occupational therapy modalities in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
    • Supports and maintains a culture of safety and quality.
    • Educates the patient and/or family in effective pain management modalities.
    • Complete patient documentation on time.
    • Attends in-service and staff meetings as needed.
    • Completes assigned training on time.
    • Completes annual education requirements.
    • Maintains patient confidentiality at all times.
    • Performs other duties as assigned.

Qualifications

 

            • Current valid state occupational therapy license.
            • Graduate of an accredited occupational therapy curriculum/school; or,
            • For Medicare certified agencies:
            • Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association; or, Is eligible for the National Registration Examination of the American Occupational Therapy Association; or,Has two (2) years of experience as an occupational therapist, and has achieved a satisfactory grade on a proficiency examination conducted, approved, or sponsored by the US Public Health Service except for those individuals initially licensed by the State or seeking qualification as an occupational therapist after December 31, 1977.
            • Two (2) years of work experience.
            • Prior home health experience, preferred.
            • Valid state driver’s license and reliable automobile.
            • Proof of current automobile insurance.
            • Current TB testing.
            • Criminal background check.
            • Current CPR card.
