Our LPNs and CPs give physicians an essential tool to deliver medical care and social support in the home, enable seamless care coordination, and extend continuity of care driving the reduction in the emergency room and hospital service utilization, improved patient outcomes, and reduced health care costs for patients and providers alike.
About the Role:
We are looking for a highly motivated, compassionate, Licensed Practical Nurse that will function to help drive our vision for the future of urgent care in the Greater Chicago area. LPNs will operate on a 12-hr shift schedule. The LPNs are responsible for delivering in-home health services and patient monitoring as directed by our medical protocols, physician, and advance practice provider partners. LPN's are provided with specialized training, equipment, and supplies required to respond to routine and urgent situations. They are expected to approach each patient encounter with a high degree of empathy and compassion. The role will require a patient-first customer service mentality and an ability to communicate clearly and concisely with patients and other team members. We are looking for someone to join our team on a Full-Time Basis and EMS experience ideally but if you have some part time availability, let us know!
Responsibilities:
• In-home assessments, screenings, and ongoing home visits under physician direction
• Conducting and documenting medical history, medication inventory, and physical exam
• Conducting and documenting home safety inspections
• Administering medication (incl. injections/vaccinations)
• Appropriate wound care
• Point of care testing and phlebotomy for commercial lab testing
• Starting IVs and administering IV medications (Will train if not currently certified.)
• Foley catheter and PEG care
• Coordination with Medical Control and patient care team (incl. physicians, PCAs, specialists, etc.)
in all patient care to deliver treatment under specified and approved protocols
• Expedient coordination with EMS and ER personnel as necessary to arrange emergency care
• Conducting appropriate in-home diagnostics incl. ECG, PulseOx, blood glucose spirometry, etc.
• Operating and maintaining company equipment per applicable safety rules and
regulations, company policies and procedures, and federal, state, and local laws/statutes
• Delivering high-quality patient care with compassion and empathy
• Maintain all required licensure and certification in good standing on an ongoing basis
Required Qualifications:
• LPN Certification with prior experience as an LPN in a direct patient care role
• Valid driver’s license with an acceptable driving record
• State-required BLS
Preferred Qualifications
• Advanced Medical Life Support (AMLS), CT, CCP training
• EMS/Paramedic Experience Preferred and/or strong Home Health Experience
• Strong communication and interpersonal relationship development skills
• Prior experience delivering in-home patient care, community paramedicine / mobile integrated
health experience, or urgent care experience working in EDs, or primary care clinics
• Prior experience working in an interdisciplinary care planning team focused on providing care
for frail, complex patients
• Ability to work independently, problem solve efficiently and execute required tasks with
strong attention to detail
Pay & Benefits Overview:
• Target Compensation Guidance
$50,430 - $74,444 depending on experience level based on full time schedule
LPNs will operate on a 12-hr shift schedule
